Referral Instructions for Providers 

Our office is currently accepting new patients and is in network with most insurance companies. All referrals are reviewed and approved by a physician before an appointment can be made. The review process can take a minimum of two weeks. Once the physician approves the referral, we will contact the patient to schedule an appointment and will send your office a fax notifying you of the appointment date and time. Please be aware our office requires a $50.00 reservation fee for a patient to make an appointment with our office. Their debit/credit card is not charged unless they “No Show” for the appointment. We explain this to the patient upon scheduling and some patients will decline the appointment because of this requirement. At that time we will notify your office.

 

Here are a few tips to expedite the referral process.

 

The following should be included with the completed referral.

• Patient demographics

• Insurance information including both primary and secondary insurance

• Office notes pertaining to the dx on the referral form

• Diagnostic testing such as labs, X-ray reports, Dexa, MRI, CT and other documents that support the reason for referral.

 

If we receive an incomplete referral or partial documentation it will significantly delay the review process.
